
NEW PATIENT INFO
Once you schedule an appointment, you will be required to complete the online intake questionnaire that is sent to you via email. You will add your insurance information, payment method and medical history in this intake form. We also ask that you to bring your driver’s license, imaging studies/reports and any particular clinical records from physicians that you have seen prior to your first visit so we can obtain a copy. This clinical information will help us be as thorough as possible.
INSURANCE
Depending on the insurance plan you have selected to use, you may need a referral from your primary care provider (PCP). If you fail to have a referral obtained from your primary care provider, your insurance will deny the claim and you will be billed. Please understand that it is ultimately your responsibility as a patient to ensure that your insurance covers chiropractic services. If you have a question in regards to your specific health insurance plan, you may call Member Services on the back of your insurance card to confirm your chiropractic therapy benefits. Dr. Anderson are Dr. Reiff are both participating with listed insurances below. (Dr. Bhatt participates in BCBS and UHC):

SELF PAY
If you do not wish to use your insurance OR do not have health insurance, self pay rates are as follows:
Chiropractic Treatments:
- Initial evaluation (up to 60 min): $150 |
- Follow up visits: 30 mins: $90 or 45 mins: $120
Massage:
- 30 mins: $60 | 60 mins: $120 | 90 mins: $180 | 120 mins: $240.

ASKED QUESTIONS WHAT SHOULD I EXPECT ON MY FIRST VISIT?
Your doctor will take a detailed history and then perform a thorough examination. A preliminary diagnosis will be rendered and a treatment plan will be recommended which is specific to you as an individual. Our goal is to resolve your condition in a timely fashion and avoid future injuries.
WHAT SHOULD I WEAR FOR MY VISIT?
It is recommended that you wear clothing that is best suited to allow for full range of motion with the ability to apply treatment to the skin.
HOW LONG ARE THE VISITS?
New patient visits are typically 45-60 minutes, which will include the history, examination and treatment. Follow up visits can range anywhere from 30-45 minutes.
CANCELLATION POLICY:
We require at least a 24 hour notice for cancellations. A late cancellation fee of $65 will be charged for cancellations with less than 24 hours notice. A no show/no call fee of $120 will be charged if no notice is given and you do not show up to your appointment.
